Obtaining a referral
In order for us to provide the best care for your patients, we need the following details:
- all relevant consultation and follow-up letters
- pathology reports
- operative notes
- procedure notes
- all X-rays relevant to the patient's medical condition
- X-ray reports
Ideally, we would prefer to have as much information about your patient, ahead of the first consultation visit. If this is not possible, then please ensure that the patient brings any information along with them.
Please fax a referral letter to the relevant practice at which you wish your patient to be seen.

Electronic referral form
The electronic referral form is a straightforward, easy to use, referral template developed for physicians and surgeons referring to Genesis CancerCare Queensland. Specialists are encouraged to use this simple referral form as it alleviates any legibility issues. The form can be emailed or faxed direct to the radiation oncologist or the patient can bring the form with them to their first appointment. Please note the electronic referral form must be signed by the referring doctor.
The electronic referral template includes:
- Referrer and Referee information (including information necessary for secure electronic referral)
- Preferred centre for treatment
- Patient Information (basic patient demographic information)
- Reason for referral
We would recommend that patients come with family members or friends, particularly if English is a second language. They may also find it helpful if they are told how to get to the practice, and what to expect on the first visit.
